Our Community Disability Services Explained

What is it? 

Community Participation is highly recognised as being important for bringing a greater sense of wellbeing, independence and inclusion to individuals.

The NDIS creates an allowance for this under their Core Support funding category, therefore it is a highly flexible funding allowance which provides opportunity to focus on the hobbies and goals of greatest interest to you. 

St Jude’s Disability services and team of Support Workers provide disability support services Perth wide as well as in WA’s South West. Each day, our team accompanies and assists clients to take part in a great range of activities that align with their interest, goals and hobbies. 

When we commence working with clients, we take the time to know you and learn about what you enjoy and what skills you are wanting to build. This in turn provides us with endless opportunities to work together to ensure you are able to take part in these activities without prevention to access.

In many cases, we assist our clients with transportation to safely arrive at their activity of choice. St Jude’s have access to accessible vehicles and can also accompany clients and help to build skills surrounding the use of public transportation. 

What is our on-boarding process?

When you first enquire about our community disability support services in Perth, we ask you about the type of support and activities you are looking to gain. This allows us to pair you with a Support Worker who is compatible with you, so you can be completely comfortable, confident and build long lasting relationships.

We then work with you to determine when the best time to receive support is and how much you require. At St Jude’s, we provide support both after hours and on weekends, which means you have full choice and control around when you would like to receive our services. This is great if you are looking to take part in more activities on the weekends or perhaps take part in a hobby in the evenings.

We continue to provide support and monitor your progress toward your NDIS plan goals. We keep these goals top of mind to ensure that whilst spending time together, you are on track to build the skills you have set out to achieve and gain greater independence in the tasks most important to you.

Our disability Support Services in Perth WA are available to all metropolitan suburbs. St Jude’s Disability Services has expanded over the years to ensure we are conveniently located and in order to grow more knowledgeable about the local communities you reside in. This best positions us to recommend activities, community groups and organisations that you may find interest in that align with your goals.

We provide community services both at your home, school, workplace or within the community. With transport services available, there’s no limitation to the types of activities you can participate in and locations that you are able to visit. 

Why St Jude’s Disability Support Services Perth

With so many disability service providers, it can be hard to determine which is right for you. The most important factor to consider is whether your provider understands you, your plans and goals and is able to deliver supports which actively align with these, while maximising your independence, choice and control. 

At St Jude’s, we pride ourselves on our flexible service model, which puts clients first. The concept of individualised services is something our entire team aim to deliver, whereby we determine the needs of each client, ahead of limiting clients to the constraints of services offered.

Our Disability Service team is available to provide support after hours and on weekends at a multitude of different locations. We also do no require a minimum NDIS funding amount, meaning we open our doors to all clients who require support and companionship, whether its for a couple hours a week, a couple hours a day or for 24/7 support.
