Mainstream Capacity Building

St Jude’s is proud to deliver the Mainstream Capacity Building program, supported by a grant from the Australian Government Department of Social Services. This innovative initiative aims to reshape the delivery of mainstream services to be more inclusive and accessible for people with disability and psychosocial needs in the remote Southwest of Western Australia.

Empowering individuals through Mainstream Capacity Building

The MCB program focuses on removing barriers to service accessibility, building individual capacity, and strengthening connections between local services. By addressing challenges unique to remote areas, the project supports individuals through all stages of transition, enabling them to access essential services, including those under the NDIS. Guided by person-centered planning, the program also explores alternative service delivery methods, such as digital platforms, to ensure long-term, sustainable solutions.

Services for Accessibility & Community Inclusion

Our Mainstream Capacity Building (MCB) program offers a range of impactful services to  improve accessibility and inclusion for individuals with disabilities:

 

  • Strategic Planning
    Creation of practical solutions to overcome barriers and improve transition processes.
  • Transition Support
    Mental health and community support and consultation with allied health therapists facilitates a safe, structured move from hospital to community living.
  • Collaboration
    Partnerships with the Department of Health, APM, Western Australian Country Health Services, Local Area Coordination providers and local organisations enhance service delivery.
  • Innovative Research
    ‘Map and gap’ exercises identify roadblocks to service accessibility and barriers to successful longterm transition back into the community for people with disability leaving hospital.
  • Sustainable Outcomes
    Training and resources ensure ongoing impact, reducing hospital readmissions and promoting coordinated discharge processes.
