Our experience in service delivery is reflected through 39 years of providing quality services and maintaining the highest of standards for our clients. About the RoleSpecialised Transition Support Staff are experienced support workers who's roles have an emphasis on working with clients with psychosocial support requirements. The role works within our Mainstream Capacity Building (MCB) team and aims to reduce the readmission cycle to hospital for clients, as well as focusing on barrier minimisation strategies for people living with Disability in the South West. Key focuses for the role include:Assisting clients to live independently and to access supports and services.Developing support plans based on the needs of individual clients.Monitoring the progress and providing reporting on support plans.Liaising with the families and representatives of clients.Seeking feedback and ensuring this is used to improve services.This role works at our Mainstream Capacity Building transition house in Bunbury. We are searching for a candidate who is able to work several day shifts (0830-1715) per week.   Skills & Experience Our ideal candidate will have previous experience working in the mental health sector or with clients requiring significant psychosocial support or have recently completed qualifications in a mental health field. They will be a proactive, positive and communicative person who prides themselves on providing quality support services to people who require them. They will have a strong ability to build meaningful, lasting relationships on a foundation of trust and respect. They will be a proficient problem solver who has a confident attitude towards difficult situations and works collaboratively with people to achieve favourable outcomes and results.    As well as the above, the Key Requirements include:Possess or willing to obtain a Working With Children Check and an NDIS Clearance Check.Relevant qualifications in mental health and/or experience within a mental health role.  Previous experience working within the NDIS or familiarity with the NDIS framework is highly desirable. Culture & BenefitsYou will be guided and supported by the manager for our Mainstream Capacity Building Program, who is based in Bunbury, and work closely with the rest of the team.
