As we get older, we may want to stay in our own homes, as this means you can remain as independent as possible and continue to make your own decisions about things that affect your life, health and wellbeing. Here at St Jude’s we understand that there is no place like home. 

St Jude’s is an approved provider of the Home Care Packages Program, and so in order to access our services, we recommend that you have access to this subsidised aged care assistance. If you are eligible, then you will receive a Home Care Package, which is a mix of services like personal care, equipment and home modification – all tailored to your needs. Your package will subsidise our aged care home support services, and make them more accessible. If you do not have a government-funded Home Care Package, don’t worry – we may have alternative cost-effective options for you. Home care provides you with extra assistance to stay safe, comfortable and well at home, and so we aim to make these services accessible for everyone.

Need More Information?

Home Care provides you with a wide range of support services that help you, or the person you care for, to continue living at home. You can receive home care as little as 30 minutes a day for a range of support services from domestic support to complex nursing and therapy services.

You may eligible for government supported Home Care which means that you may not need to pay anything. Speak to your GP about your support needs.

The GP can refer you to an Aged Care Assessment Team, or if you are in hospital the medical and nursing staff can arrange this for you, or you can ring ‘My Aged Care’ on 1800 200 422.

If you do not have a GP or in a crisis situation give us a call on 9279 4343.

The Aged Care Assessment team may meet with you in person at your home. With your consent they will assess your care needs and eligibility for services. They will then work with you to develop a support plan which addresses your needs and preferences.

You will receive a letter outlining your eligibility for a package and approved the level of package and funds that has been allocated.

Once you have received the results of your assessment, you can contact us to discuss the services you would like to receive.

We can also arrange top-up services if the funded support does not meet your support needs.

Our representative (Case Manager) will come to you and discuss the services we can offer. If you choose us, we will carefully design a plan that best suits you. You may ask your friends or families to join the meeting.

With the information you provided, the Case Manager will prepare a service plan, proposed budget and a home care agreement.

These will be given to you for you to discuss with your family, carer or your friends to make sure you have everything you need.

When you are ready give us a call and the Case Manager will return to meet with you to sign your Home Care Agreement, complete a full assessment, prepare your care plan based on your assessment, and confirm your service plan.

We will provide you a file with all relevant information to keep at your home.

Our schedulers will then put in place and assign your services based on the dates and times you wish to commence with St Jude’s Home Care Services.

Fees
